* Please Note: You can submit this form but you must submit the original, signed, form - with the corresponding Form Number - within 3 days of the fixture having been played. For example, you submit the form online on Friday night, the player (providing he is not suspended or registered to another Club) is eligible to play on Sunday. You must ensure the original form is with the Registrations Secretary on or before Wednesday after the match. Failure to do so shall result in the Club being dealt with as having played an ineligible Player. This could involve a heavy fine and point(s) deduction. Entering incorrect details may result in the registration being cancelled and the Club being dealt with as playing an ineligible Player.
Please ensure the correct Form Number is entered. Any incorrect information may result in the Club being fined and/or the registration of the Player being cancelled.
